What SHBG Means

SHBG stands for sex hormone-binding globulin. It's a protein made mostly by the liver that attaches to testosterone and carries it through the bloodstream. Only a portion of your total testosterone circulates freely and is considered readily available to tissues; the rest is bound to SHBG or, to a lesser degree, to albumin. Because of this, SHBG levels influence how much testosterone is actually usable at a given moment, even when total testosterone looks normal on paper.

Why SHBG Comes Up During Evaluation

When a clinician reviews hormone levels, total testosterone alone doesn't tell the whole story. Two people with the same total testosterone number can have very different amounts of free, usable hormone depending on their SHBG level. That's why SHBG is often discussed alongside total and free testosterone measurements — it helps put the bigger picture into context rather than being interpreted as a stand-alone result.

What Can Influence SHBG Levels

SHBG can shift for a variety of reasons, including age, liver function, thyroid activity, body composition, and certain medications. Because so many factors can move this number up or down, an individual reading is generally reviewed alongside other labs and personal history rather than judged in isolation. This is one of the questions to discuss during a consultation — how your specific SHBG level fits with your other results and symptoms.

How This Connects to a Testosterone Therapy Program

SHBG is one of several lab markers that may be part of a broader hormone evaluation. If you're exploring Evaluation & Lab Review, understanding SHBG can help you ask more informed questions about how your results are being interpreted together, not just individually.

FAQs

Is SHBG the same as testosterone?

No. SHBG is a protein that binds to testosterone in the blood; it's not testosterone itself. It affects how much testosterone is considered freely available rather than bound and inactive.

Can SHBG be too high or too low?

Yes, SHBG can run higher or lower than typical ranges for a variety of reasons. Whether a specific level is meaningful for you is something a clinician would evaluate alongside your full history and other lab results.

Will SHBG always be tested during a testosterone evaluation?

Whether any particular lab, including SHBG, is ordered depends on individual clinical judgment. A consultation is the appropriate place to ask what markers may be relevant to your situation.

Does a high SHBG level mean testosterone therapy won't work?

SHBG is one piece of a larger picture. It doesn't independently determine whether a program may be appropriate — that assessment involves reviewing your full history and results together during individual clinician review.

How does SHBG relate to free testosterone?

Free testosterone refers to the portion not bound to SHBG or albumin. Because SHBG binds a large share of circulating testosterone, its level can meaningfully affect free testosterone calculations or estimates.

Can lifestyle factors change SHBG levels?

Body composition, thyroid function, and certain health conditions are among the factors that can influence SHBG over time. Specific causes and relevance to your case are best discussed individually.

What should I do if I have questions about my SHBG result?

The best next step is to request a consultation so a clinician can review your specific results and history and explain what they may mean for you.
